Nicholas Brendon, best known for his role in Buffy the Vampire Slayer, had his cause of death officially detailed weeks after his passing. The actor died in March at age 54, and new findings from the autopsy report are now offering a clearer picture of what led to his death.

According to the autopsy report obtained by Page Six, Brendon died from a combination of serious heart-related conditions. The report lists atherosclerotic and hypertensive cardiovascular disease as the primary cause, with acute pneumonia and a previous heart attack contributing. Officials said his death was natural, pointing to a severe blockage, about 90 percent, in a major coronary artery.
Putnam County Coroner Todd Zeiner explained the findings in detail, noting the actor had “a markedly enlarged heart” along with narrowed arteries and lung infection. He also mentioned inflammation in the small bowel, likely tied to reduced blood flow connected to “cardiogenic shock.” These combined issues painted a picture of long-term health struggles.
Brendon was found at his Indiana home by his manager and close friend, Theresa Fortier, who discovered him on a couch in what appeared to be a “sleeping position.” Authorities noted there were clear signs of death when she called emergency services, and no attempt at resuscitation was made. Officials estimated he passed away earlier that morning.
The report also highlighted symptoms Brendon had been dealing with before his death. He had been coughing persistently and complained of chest pain, but chose not to seek hospital care. “He had been having a persistent cough,” Zeiner noted, adding that the actor was treating himself with over-the-counter drugs.
Brendon’s health issues were not new. He had undergone spinal surgeries after a fall and previously suffered heart complications, including a heart attack in 2022. Despite personal struggles, his family remembered him as “passionate, sensitive and endlessly driven to create,” while asking for privacy as they mourn his loss.
